 Preservation of Natural Frontal Sinus Outflow with subdural stent
Preservación del tracto de salida del seno frontal mediante stent subdural

SUMMARY

Introduction: The frontal sinus remains one of the most complexes sinonasal regions to operate, representing a challenge for even the most experienced surgeons. Recently, stents created to prevent that two raw surfaces come into contact, seem to improve this decision. In this study we show our results in subdural stent placement in the outflow tract of the frontal sinus to solve their chronic disease. Material and methods: Retrospective review of the medical and surgical records of patients implanted with subdural sinus stents placed between April 2009 and April 2013 were revised. Results: Over a period of 4 years, 15 stents were implanted in the frontal sinus. Each patient had a single stent placed, except two who were placed double. At the time of surgery, the average age was 45 years. In eleven patients were placed the stent by endoscopic approach alone, the rest was a mixed approach. The stent length was 18.4 months (range 3-33 months). In four patients the stent was removed due to infection, pain, granuloma formation and/ or frontal region edema. The other patiens remain asymptomatic. Discussion / Conclusion: In our group, placing this new type of stent in the frontal sinus is associated with significant improvements in the control of chronic relapsing disease. The amount of withdrawals stent is probably related to the internal diameter of these stents. Maintaining the TSSF stent for long periods of time can cause unwanted consequences, such as foreign body reaction, requiring the withdrawal. Patients should be followed regularly as a possible complication is stent migration with consequent stenosis of the ostium.

RESUMEN.

Introducción: El seno frontal continúa siendo una de las regiones nasosinusales más complejas para operar, representando un reto para incluso los cirujanos más experimentados. Recientemente, los stents creados para evitar que dos superficies cruentas entren en contacto parecen mejorar dicho fallo. En este estudio mostramos nuestros resultados en la colocación del stent subdural en el tracto de salida del seno frontal para poder resolver su patología crónica. Material y Métodos: Estudio retrospectivo donde se revisan las historias clínicas y registros quirúrgicos de pacientes con stents subdural en seno frontal colocados entre abril de 2009 y abril de 2013. Resultados: Durante un período de 4 años, 15 stents fueron implantados en el seno frontal. Cada paciente se le colocó un único stent, exceptuando a dos que se les colocó doble. Al momento de la cirugía, el promedio de edad fue de 45 años. A 11 pacientes se les colocó el stent mediante abordaje endoscópico exclusivamente, el resto fue un abordaje mixto. La duración del stent fue de 18.4 meses (rango 3 a 33 meses). A 4 pacientes se les retiro el stent debido a infección, dolor, formación de granuloma y/o edema de región frontal. El resto permanecen asintomáticos Discusión/Conclusión: En nuestro grupo, el colocar esta nueva modalidad de stent en el seno frontal está asociada a mejoras significativas en el control de patología crónica recidivante. La cantidad de retiros de stent probablemente esté en relación al diámetro interno de estos stents. Mantener stent en TSSF durante largos períodos de tiempo puede originar secuelas no deseadas, como reacción a cuerpo extraño, que obliguen a la retirada del mismo. Los pacientes deben seguirse con regularidad ya que una posible complicación es la migración del stent con la consecuente estenosis del ostium.
